A lot of good people put in some time to write detailed instructions on how to achieve something in the game and I think it would be a good idea to keep them in one place, where everyone can see them.

Feel free to mention any other posts that should be on here.

Story guides:

It's impossible. However, it may be become possible in Act II, and related with "you didn't let anyone die" achievement.
